Therapy Fees

Individual Psychotherapy and Play Therapy sessions are $125.00 per 45 minute session. I accept cash, debit/credit card, and HSA payments that are due prior to the start of the therapy session.

Insurance

At this time I am in network with the following insurance plans:

  • Paramount

  • Anthem Blue Cross/Blue Shield Commerical/Medicaid

  • Tri-Care East

  • Buckeye

  • Caresource

  • Cigna

  • Optum/United Health

  • Frontpath

  • UMR

  • Multiplan (with Frontpath)

  • Self-pay (out of pocket)

For other plans, I am an out-of-network provider. You are responsible for paying my fee and I will provide you with a receipt of services for you to submit to your insurance provider. Please check with your insurance provider for the details of your coverage. When contacting the provider be sure to ask the following questions:

  • Do I have out-of-network coverage for mental health?

  • Do you cover the LPCC (Licensed Professional Clinical Counselor) license?

  • What is my out of pocket payment before my deductible is met?

  • What percentage of the fee for each visit will be covered?

  • How many visits are covered per calendar year?

A Note about insurance: The benefit of not using your insurance is that your psychotherapy treatment is fully your own and completely confidential. It will be determined by you and me with no restrictions from your insurance company. In addition, a mental health diagnosis will not have to be added to your permanent health record.

Scheduling

Typically, we will set up a counseling sessions at the end of the previous session (before leaving your session). In the event that this has not happened, please contact me at (419) 458-3030 to schedule an appointment. To get the maximum benefit from therapy, I recommend at least every other week appointments. In the event that more intense services are needed, this will be communicated with you. I offer appointments Monday-Friday from 8 AM-8 PM, and Saturdays from 8 AM-12 PM.

Cancellation

I require a 24 hour notice for canceling an appointment. If less than 24 hours is given, you will be

responsible for a $25.00 cancellation fee due before the next counseling session. In the event, of

three or more cancellations, you will be terminated from services.
